Licensed in the State of Washington as a physical therapist
Health Care Provider level CPR certification
Current Washington state driver's license
Minimum of one (1) year of physical therapy experience
Experience in Medicare Certified Home Health preferred
Proof of a safe driving record (which meets established MultiCare Standards) obtained by a motor vehicle report from the appropriate state

Position Summary
The Home Health Physical Therapist is an exciting opportunity at MultiCare, responsible for evaluating, planning, and implementing medically prescribed treatment programs for patients to restore functions, relieve pain and prevent disability following disease or injury. This professional-level position gives you the capacity for independent operations as you apply discretion and good judgment toward ensuring high-quality therapeutic care. Responsibilities:
You will provide skilled physical therapy in accordance with the physician's plan of care
You will assist the physician in evaluation and assessment of the patient's functional level
You will treat patients to relieve pain, develop and restore function and maintain maximum performance
You will observe, record and report the patient's reaction to treatment and any changes in condition
You will instruct and advise patients, family and other health team members in the phases of physical therapy
You will evaluate and regularly re-evaluate
