As a Refugee - Case Manager, you will provide flexible, outreach ‑ based support to participants across Cumberland , Parramatta, Blacktown and The Hills regions. Working both in homes and the community, you will help clients develop practical life skills, access services, and navigate the Australian support system.
Responsibilities
Deliver individualised recovery plans to participants with mental health needs.
Provide advocacy, service navigation, referrals and linkages to local services .
Conduct home visits and community outreach.
Work collaboratively with families (where appropriate) , health providers and other agencies.
Maintain high ‑ quality documentation, case notes and reporting.
Promote the CLSR service within the community.
Contribute to continuous improvement and uphold Anglicare's values.
Requirements
Certificate IV in Community Services, Mental Health, BA Social Work , Counseling or similar.
E xperience supporting clients in community services, settlement, or mental health.
Experience working with multicultural or CALD communities.
Understanding of mental illness and recovery ‑ oriented practice.
A valid NSW driver's licence (pool vehicles provided).
Strong communication , empathy and relationship ‑ building skills.
