Liquid Personnel is looking for experienced Occupational Therapist to support its client's Adult Social Care service tea in Rutland, delivering high-quality interventions to enable individuals to remain independent in their own homes.
This is a varied role, combining reablement, discharge support, and community-based interventions.
What will your responsibilities be?
Complete holistic, strengths-based assessments and develop personalised reablement plans
Support individuals on Discharge to Assess pathways, enabling safe return home
Work with community patients to prevent hospital admissions
Assess and provide equipment and adaptations (including major adaptations)
Complete risk assessments, including complex moving & handling cases
Promote and support use of assistive technology
Work collaboratively with health and social care professionals
Maintain accurate case records and monitor support plans
Supervise OT Assistants and provide guidance to colleagues
