Job Description A bit more about this role: Devoted Medical, Devoted Health's virtual-first, AI-native medical group delivering multi-specialty and advanced primary care, is seeking a dynamic, experienced, and mission-driven physician leader. As the Medical Director Episodic Care and Enterprise Safety, you will be a chief clinical architect of our next-generation care delivery model in the post-ED, post-hospital and post-acute care space. You will lead the charge in transforming how acute care is delivered outside traditional hospital walls, ensuring that every patient encounter across the national acute care continuum is safe, effective, and seamlessly connected to long-term health outcomes. Your work will define the "moat" of our business-a blend of human expertise and AI-driven precision that makes our care model irreplaceable. Because Devoted is a tech-forward, fast-moving organization, our physician leaders are expected to support the development and application of AI tools that enable an unprecedented quality of care for our patients. This role is designed for a senior physician leader with an entrepreneurial mindset who can leverage their experience to transform the care model, cultivate a standard of clinical excellence within a distributed team, and build and champion the implementation of evolving AI technologies. In this capacity, the national medical director will act as the ultimate point of accountability over clinical and financial outcomes of a large and scaling business unit. Your Responsibilities and Impact will include: National Clinical Leadership Lead and scale interdisciplinary care teams that support patients across the full acute care continuum at a national scale, fostering a culture of excellence and clinical rigor. Responsible for hiring and managing the leaders of a growing team of physicians and advanced practice providers Designing performance management systems that balance impact across multiple dimensions: clinical outcomes, patient experience, staff engagement, and productivity to have impact on a national scale Episodic Care Model Ownership Drive the strategy and business success for episodic care in the post-ED, post-hospital, and post-acute phases. Instrument a reliable AI-enabled system that surfaces high value care pathways that are personalized to the patient context and , Expand the clinical aperture to avoid unnecessary reuse, and ensure durable outcomes that extend well beyond the critical 7-30 day window. This includes performance in 3 high value event based stars measures: Plan All-Cause Readmissions (PCR), Follow-up in patients with multiple co-morbidities after ED visits (FMC), and transitions of care (TRC) patient engagement. Virtual 'Observation' Program Build and scale a high-touch, virtual in-home "observation" level of care program. This program will serve as a safety net for patients at high risk of rehospitalization due to new or unresolved acute conditions. Enterprise Quality & Safety Serve as the Medical Leader for the Patient Safety Organization (PSO). Leverage deep intelligence from acute events to implement critical safety patterns and quality standards across the entire enterprise.
