Learn more at shn.ca Job Description: Position Overview: Reporting to the Patient Care Manager, you will work independently within your scope of practice when the patient's condition indicates predictable outcome and you will deliver nursing care based on a pre-set plan of care with a delineated range of intervention. You will collaborate with and get directions from the RN in situations that are beyond the scope of complexity designated to you as a RPN. You will be part of a busy department that provides excellent care to a diverse patient population within our multi-service environment who have been designated Alternate Level of Care (ALC) and require short stay intensive transitional care for discharge. The model of care on this unit is a team based model working closely with Personal Support Workers (PSW) and Registered Nurses (RN) along with allied health team. You will provide high quality nursing care to patients and their families, based on the philosophy and conceptual framework of Nursing Practice at Scarborough Health Network.
