Role Scope: We are looking for a mission-driven Registered Nurse to join our team as the RN Supervisor, Pre-Enrollment for Southern California. This is a centralized clinical supervisory role supporting Enrollment RNs across all four Southern California PACE sites. The RN Supervisor serves as the operational and clinical execution layer between frontline Enrollment RNs and the RN Clinical Manager. The RN Supervisor serves as the first clinical filter for Level 2 cases, owns day-to-day enrollment case operations, enforces SOPs, and ensures documentation quality, workflow consistency, and regulatory readiness across sites. This role is critical to scaling Pre-Enrollment operations safely and compliantly as Habitat Health expands to West Covina and San Leandro in January 2027. Core Responsibilities & Expectations for the Role Own daily review of the RN tracker, Salesforce enrollment pipeline, and Epic work queues to ensure alignment across systems Identify stalled cases, missing documentation, incomplete assessments, or scheduling gaps and return cases to Enrollment RNs with clear, time-bound next steps Ensure all case status updates are reflected consistently across Epic, Salesforce, and the RN tracker Serve as the primary point of contact for day-to-day enrollment case questions from Enrollment RNs and Intake staff Monitor completed, upcoming, canceled, and rescheduled Enrollment RN home visits across all Southern California sites Track 30-day forward-looking visit capacity to anticipate bottlenecks and staffing constraints Identify no-show trends and recommend scheduling or workflow adjustments Track applicants by enrollment readiness category (signed, DHCS-approved, pending assessment, pending state response) and identify cases at risk of missing the monthly DHCS cutoff Escalate at-risk cases to the RN Clinical Manager with specific mitigation recommendations Serve as the central operational reviewer for all ILOC submissions prior to DHCS submission; coordinate ILOC email workflows and submission sequencing Catch documentation discrepancies prior to state submission and manage resubmissions; enforce 24-hour ILOC turnaround standards and maintain Epic audit trails Receive and triage all potential Level 2 case flags from Enrollment RNs across SAC, SLA, West Covina, and San Leandro; confirm escalation criteria and ensure documentation completeness prior to escalation to RN Clinical Manager Own end-to-end Health & Safety (H&S) Committee operations for Southern California sites, including case intake and triage, documentation gap review, committee packet assembly, meeting facilitation, real-time Epic documentation, and outcome communication and compliance follow-up Author concise clinical summaries aligned with Medical Director communication standards; ensure all H&S determinations comply with 42 CFR §460.150(b)(1) and §460.152(b) requirements Enforce consistent application of all Pre-Enrollment SOPs across Southern California sites; deliver training content developed by the Director and Clinical RN Trainer to frontline Enrollment RNs Maintain training logs and competency documentation; identify recurring errors or training gaps and escalate to RN Clinical Manager Coordinate Enrollment RN schedules and home visit assignments; monitor documentation quality, timeliness, and completion rates Collaborate with Intake, Data Ops / Health Plan teams, and Clinical Operations to resolve workflow gaps Maintain full enrollment operations during short-term leadership absences, ensuring no disruption to case flow, DHCS submissions, or H&S determinations Participate in all PACE Audits by DHCS and CMS Other duties as assigned Required Qualifications: Passion for delivering amazing coordinated care to frail elders and disabled older adults Active and unrestricted RN License in the state of California, in good standing Minimum 3-5 years of clinical nursing experience; PACE, geriatrics, or home health strongly preferred Experience with I
