Responsibilities

  • The successful candidate will be able to perform the following responsibilities:
  • Engage pregnant patients throughout pregnancy to assess health-related social needs (HRSN)
  • Develop and document individualized care plans in collaboration with OB providers and the multidisciplinary care team.
  • Educate patients on navigating healthcare services, insurance coverage, and community-based resources.
  • Coordinate referrals and facilitate warm hand-offs to internal services and external community and legal partners.
  • Track referral completion and conduct follow-up outreach to ensure patients successfully access needed services.
  • Review reports to confirm patients missing initial prenatal and postpartum visits and communicate with perinatal teams regarding outreach
  • Participate in weekly high-risk OB rounds to support case review, care planning, and follow-up.
  • Document patient interactions, referrals, and outcomes accurately in EPIC.
  • Collect and track screening and referral data to support quality improvement initiatives, reporting, and grant requirements.
  • Collaborate with Information Technology and Population Health/Quality Improvement teams to support workflow improvements and documentation processes.
  • Maintain accurate, standardized documentation to support HRSN care navigation workflows and quality improvement efforts.

Requirements

  • Bachelor's degree in healthcare related field required.
  • Minimum of 1-2 years of experience in care navigation, community health, case management, or OB care coordination. Experience working with underserved populations.
  • Strong interpersonal and communication skills, with the ability to build rapport across diverse teams.
  • Exceptional problem-solving and conflict-resolution abilities.
  • Ability to handle sensitive and confidential matters with professionalism and discretion.
  • Proficiency in Microsoft Office Suite.
  • The ability to work in a fast-paced environment.
  • Spanish fluency strongly preferred; ability to communicate effectively wit
