The successful candidate will provide support and services to adults with learning disabilities, helping them to achieve their full potential and improve their quality of life. This role involves assessment, planning and intervention to address the needs of individuals and their families. What will your responsibilities be? Conduct comprehensive assessments of clients' needs and circumstances. Develop and implement personalized care plans. Provide counseling and support to clients and their families. Collaborate with other professionals and agencies to coordinate care. Advocate for clients' rights and access to services. Maintain accurate and up-to-date client records.
