Assists in the nursing process by observing and reporting changes in the residents' physical and emotional condition; provides input into the care plan and implements the guidelines in the plan
Adheres to the legislation, standards, policies and procedures of the LTC Home Delivers personal care to promote comfort and safety for the resident during activities of daily living in accordance with the standards of resident care set out in the Nursing Manuals
Provides cognitive, social, emotional support to residents.
Promotes independence of resident in his/her activities of daily living. Observes and respects resident's individual rights and choices. Acknowledges the resident's need for dignity and Privacy
Assists the resident in the proper use and care of adaptive/prosthetic devices Maintains effective communication and good relationship with residents, families and significant others
Participates as a team member in co-operation with other departments to ensure quality care of the resident. Provides input into the plan of care
Documents accurately and legibly on flow sheets and daily records reflecting each resident's condition, care and specific needs
Assists in the care of resident's personal belongings
Complies with risk management and quality improvement programs, including infection prevention and control, occupational health and safety, incident and falls, etc. Assists in the care of LTC home equipment by regular cleaning and inspection and prompt reporting of defects.
Provides job shadowing or mentorship to members of the health care team or students, as assigned
Practices nursing restorative care to complement and supplement rehabilitation care
Keeps up-to-date with developments and best practices in the field
Other duties as assigned
Requirements
Graduate of an approved Personal Support Worker course in Ontario, or equivalent
Experience in long term care setting an asset
Current CPR Certificate
Registration with Health and Supportive Care Providers Oversight Authority (HSCPOA) preferred.
Resident-centred care focus with strong customer service and interpersonal skills
Demonstrated ability to take initiative, problem-solve and time manage
Ability to work independently as well as be an active, positive team player
Behavioural management training (Gentle Persuasive Approach (GPA), U-First, CPI) an asset
Computer literacy (Point Click Care, MS Office). Basic knowledge of Workday an asset.
Ability to speak and converse in Chinese an asset
Working Conditions:
Considerable standing, walking, lifting and bending
Exposure to all resident care elements through regular resident contact.
Ability to work permanent day shift, including weekends and short shifts. May be required to work an alternate shift(s) to meet operational needs (e.g., emergency, training, accommodation, etc.) as per scheduling practices
