BE/BC Family Medicine or Internal Medicine & Pediatrics
Current medical license in the Commonwealth of Massachusetts, or ability to obtain
Must demonstrate integrity, sound judgment, demonstrated leadership skills, and strong interpersonal skills.
Experience working with disadvantaged populations and knowledge of health disparities preferred.
Excellent oral and written communication skills needed, strong organizational ability required.
Prior experience implementing and/or leading population health programs/initiatives preferred
Outstanding skills in data collection, analysis, and presentation. Experience in the use of spreadsheets.

In Partnership with the Senior Director of Population Health, the Medical Director of Population Health is responsible for helping guide the overall direction, coordination, and implementation of the Population Health Program for the Sturdy Health Medical Group (SHMG).

Partner with the Senior Director of Population Health to develop, operationalize, and communicate initiatives aimed at meeting and exceeding quality metrics set by our ACO partners and insurers.
Utilize the clinical experience and expertise, in collaboration with clinical experts and stakeholders, to facilitate founding principles of Population Health
Inspire others to live our mission by regularly and actively soliciting new Population Health ideas
Collaborate with SHMG leadership to explore and bring forth strategic initiatives
Assist in the development, monitoring, and presentation of internal quality measures and initiatives
Develop long-term relationships with an existing patient base
Implement a proactive approach focusing on preventative medicine
Collaborate with other medical and surgical specialties
