Essential Responsibilities
Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations.
Assessment: Collect comprehensive data pertinent to the patient's health or situation.
Diagnosis: Analyze the assessment data to determine the diagnoses or issues.
Outcomes Identification: Identify expected outcomes for a plan individualized to the patient or the situation.
Planning: Develop a plan of care that prescribes strategies and alternatives to attain expected outcomes across the care continuum for inpatient and/or outpatient procedures.
Implementation: Implement the identified plan.
Evaluation: Evaluate progress toward attainment of outcomes and revise as needed to meet goals.
Quality of Practice: Systematically enhance the quality and effectiveness of nursing practice using the Relationship Based Care model and shared governance for decision making.
Education: Attain knowledge and competency that reflects current evidence based practice for nursing care. Facilitates health and disease patient education, including leading group office visits. Supports patient self-management of disease and behavior modification interventions.
Professional Practice Evaluation: Evaluate personal nursing practice in relation to professional practice standards and guidelines, relevant statutes, rules and regulations. Promotes clear communication among a care team and treating clinicians by ensuring awareness regarding patient care plans.
Collegiality: Interact with and contributes to the professional development of peers and colleagues. Participates on a team for data collection, health care outcome reporting, clinical audits, and programmatic evaluation.
Collaboration: Collaborate with patient, family, and others in the conduct of nursing practice.
Ethics: Integrate ethical provisions in all areas of practice. Utilizes the ethic decision making model to drive issues and advocate for patient wishes.
Research: Integrate research findings into practice. Monitor the progress of patient through the service plan and re-evaluate the effectiveness and appropriateness of the plan based on the patient's condition and needs.
Resource Utilization: Consider factors related to safety, effectiveness, cost and impact on practice in the planning and delivery of nursing services. Provide patients linkages to services to promote a pathway to care and positive outcomes.
