Responsibilities
Provides individuals, family, and group counseling to persons with diverse diagnosis for senior/disabled adults.
Development of plans of care Implementation .
Work as part of a multidisciplinary team to ensure high quality outcomes for patients.
Patient assessments and creative engaging activities to enhance group therapy .
Assist patients in obtaining services to enhance their health and wellbeing.
Requirements
Masters degree in Counseling / Social Work .
1-3 years experience in Clinical Therapy
Licensure required LCSW or LPCC
