Job Overview Job Description The Activities Coordinator is a member of the PACE Interdisciplinary team (IDT) and is responsible for actively participating in all care coordination activities, including Assessments and Care Planning. This role involves designing, implementing, and overseeing a variety of engaging activities tailored to the physical, cognitive, social, spiritual, and social well-being of the PACE participants. The Activities Coordinator is responsible for utilizing a variety of techniques, including but not limited to arts and crafts, animals, sports, games, dance and movement, music, and community outings. The activity program shall also provide a planned schedule of social and other purposeful independent or group activities and promote an engaging environment for the participants. The Activities Coordinator may use remote technology depending on the participants' comfort level, electronic devices, and interests. The Activities Coordinator will be responsible for adhering to PACE program guidelines and regulatory requirements, adhering to the PACE organization's policies and procedures, and best practices. This position is under the guidance and direct supervision of the PACE Care Center Supervisor at PACE centers. Minimum Requirements Minimum of a High School diploma or equivalent. A bachelor's degree is highly desirable. Minimum of 1 year of experience in a social or recreational program providing and coordinating services for a frail or elderly population (preferred). Experience with dementia care is highly desirable. Completion of 36 hours of Activity Leader (previously "Activity Director") course, designed specifically for this position and approved by the State of California, is required within 30 days of hire. Bilingual English/Spanish/Mandarin/Cantonese, depending on location preferred. Experience in providing virtual activities is preferred. A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.
