Description of Work This position is responsible for leading team meetings and work groups of families of children and youth with special health care needs (CYSHCN) to work on the development and maintenance of effective programs for CYSHCN. The Family Liaison Specialist will be responsible for assisting families of CYSHCN with information and referrals through a toll-free Help Line for CYSHCN as requested. This position will assist in implementing and monitoring a contract with an external agency to provide timely reimbursements for family members. Additionally, the position will guide the Whole Child Health Section staff as to family needs and work with the development of family-centered programs. The position will also provide support to the Innovative Approaches Initiative, a systems change initiative focused on CYSCHN, as requested. This position trains, assists and provides guidance to staff on family related activities including, but not limited to, family perspectives, family centered care, care coordination, transition planning, medical home, and educational/community resources. The position staffs teams and work groups of families to assure Whole Child Health Section programs and services are family centered.
