As an Inpatient Behavioral Health Case Manager on our close-knit psychiatric care team, you'll play a vital role in coordinating patient-centered care while ensuring no one carries a patient alone - collaboration and communication are at the heart of how we work.
Clinical Responsibilities
Interviews and assesses patients and/or patients' families, caregivers, and/or legal representatives to identify clinical, psychosocial, and resource needs
Determines, prioritizes, provides, and/or arranges for appropriate internal and external services and interventions
Consults regularly with multidisciplinary teams to evaluate patient status, treatment progress, and appropriateness of medical and behavioral health care across care settings
Performs ongoing chart review to identify actual or potential issues related to service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement
Care Coordination & Discharge Planning
Actively participates in discharge planning, including identifying and coordinating appropriate post-discharge resources
Provides discharge education and resource referrals to patients and families to support safe transitions of care
Coordinates care for an inpatient psychiatric population, including patients undergoing alcohol and opioid detox
Requirements
Experience in behavioral health, care management, or related clinical setting
Must hold a Maine State license as a Licensed Social Worker (LSW OR LSX), Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or Registered Nurse (RN)
Strong assessment, communication, and care coordination skills
Ability to work collaboratively within a multidisciplinary team
A patient-centered, compassionate approach to care management in an inpatient setting
