Job Summary: The Rural Health Liaison conducts outreach activities and serves as a resource for members, providers, and community-based organizations (CBOs) to address the diverse health needs of rural populations. This role leads outreach initiatives, facilitates education and support programs, and contributes to policy development aimed at promoting health equity and enhancing health outcomes in rural communities. Essential Functions: Engage with members, providers, and community-based organizations (CBOs), and other stakeholders to share information, resources, and provide education on health access and services Provide proactive, high-level account development and support in order to drive the overall satisfaction and engagement of key rural health partners Handle, refer, or delegate complex or non-routine questions, issues, and problems to appropriate resources as they relate to the assigned accounts and ensure they are resolved in a timely manner. Timely is defined as 30 days or less. Maintain organized files and document all interactions in the Customer Relationship Management tool ("CRM") Support recruitment and contracting efforts by developing strong interpersonal relationships with providers, hospitals and ancillary providers Promote access to care by connecting rural community members to healthcare services, benefits, programs, and resources, ensuring they are informed about available options that address their unique health needs Conduct outreach activities, including presentations and providing educational materials, to increase awareness about health issues and promote healthy behaviors Collect and analyze data and insights on specific health needs and challenges faced by rural communities to identify outreach opportunities Provide input for policy development and support the implementation of innovative solutions to the meet the unique needs of rural communities Foster, coordinate, and facilitate cultural competency training for CareSource staff Provide support and resources to providers and CBOs to enhance efforts and address gaps in care and health literacy Organize and host community townhalls, rural member/provider/community advisory groups, listening sessions and other events to inform, educate, gather data and information Establish relationships among rural community-based organizations, stakeholders and providers to stay abreast of rural health challenges and opportunities Collaborate with Managed Care Organization (MCO) to address social determinants of health and integrating wellness and holistic health Promote MCO plan services and benefits to improve the health equity in rural communities Develop comprehensive success metrics that demonstrate the impact and value of Rural Health initiatives and outreach to members, providers, and CBOs Performs other job related instructions as requested Education and Experience: Bachelor's degree or equivalent years of relevant work experience is required, preferably in social work or public health Minimum of three (3) years of health care, public health or community based organization experience or experience related to the responsibilities of the position is required Familiarity of rural health communities, rural health disparities and knowledge of Medicaid preferred Competencies, Knowledge and Skills: Proficient in Microsoft Office especially, Word, PowerPoint, Excel and Access Fluency (speaking, reading, and writing) in multiple languages including English and Spanish is preferred Sensitivity to and experience working within different cultures Ability to work independently and in a team environment Knowledge of community and state support resources Ability to work with difficult situations with multiple interests/parties involved Strong planning, organizational, multi-tasking skills Strong interpersonal skills, including active listening and relationship/trust/consensus building Research and analytical skills Demonstrated program development skills Excellent written, verbal and presentation skills Spanish speaking preferred Licensure and Certification: Community Health Worker certification is preferred Must have valid driver's license, vehicle and verifiable insurance. Employment in this position is conditional pending successful clearance of a driver's license record check and verified insurance. If the driver's license record results are unacceptable, the offer will be withdrawn or, if employee has started employment in position, employment in the position will be terminated.
