Responsibilities
- In this role you will be responsible for:
- Conducting medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment
- Determining follow-up assessments according to patient needs and best practice guidelines
- Communicating significant findings, problems, and changes in condition or environment to the clinical coordinator, the physician, family, facility, and other personnel involved with patient care
- Assessing health status by obtaining relevant health and medical history
- Performing a physical exam based on the age and patient history as per the Standard Care Arrangement with a collaborating physician
- Ordering, interpreting, and evaluating laboratory tests and other diagnostic studies to assess patient's clinical problems and healthcare needs

- Prescribing and monitoring medications as allowed by prescriptive authority through the appropriate State Board Agency
- Developing a treatment plan in collaboration with the patient and caregivers
- Referring patients to physicians as appropriate for consultation or for specialized health resources and treatment
- Ensuring continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas
- Providing patients and families an understanding of the diagnosis and disease progression while decreasing unnecessary emergency room and hospital visits
- Holding case conferences with families when appropriate to advance the care plan
- Meeting minimum productivity requirements based on practice standards.
- Required Qualifications
- Master's or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant
- Current State License as a Nurse Practitioner or Physician Assistant
- Sit, stand, bend, lift, and move intermittently
- Must be able to lift at least 35lbs
