Liquid Personnel is currently recruiting for a Social Worker to join a Community Team based in Cheshire East.
What will your responsibilities be?
Conduct comprehensive assessments to determine the needs and risks of individuals.
Develop and implement care plans in collaboration with service users, families, and multidisciplinary team's
Provide guidance and support to individuals, empowering them to access services and maintain independence.
Advocate on behalf of service users to ensure their rights and needs are met.
Maintain accurate records in compliance with local policies and legislation.
Work closely with healthcare professionals, voluntary organizations, and community groups to coordinate care.
Ensure safeguarding principles are upheld, identifying and responding to concerns appropriately.
