Provide occupational therapy services to patients in their homes throughout Flathead County and Lake County.
Implement individualized treatment programs according to occupational therapy principles, Logan Health policy, payer requirements, and Montana practice guidelines.
Collaborate with patients, families, caregivers, and interdisciplinary team members to establish goals and support discharge planning.
Complete required documentation in a timely manner according to Medicare, Medicaid, private insurance, payer guidelines, and Montana practice standards.
Communicate effectively with patients, families, peers, students, and care team members regarding patient care and progress.
Manage and self-schedule patient visits while carrying a caseload of up to 25 patients.
Meet productivity standards and department expectations.
Participate in department, facility, and community projects or programs that support and enhance rehabilitation services.
Maintain regular and consistent attendance as scheduled by department leadership.
Perform other duties as assigned within the scope of the role.
Requirements
You are a licensed or soon-to-be licensed Occupational Therapist who is passionate about helping patients improve their quality of life. You are organized, independent, compassionate, and comfortable providing care in a home health setting.
You bring strong communication skills, sound clinical judgment, and the ability to build trust with patients and families. You enjoy working as part of a team while also having the confidence to manage your own schedule, patient visits, documentation, and caseload of up to 25 patients.
Current Montana Occupational Therapist license required.
Graduate of an accredited Occupational Therapy program with the ability to obtain a Montana Occupational Therapist license within 30 days of hire.
RQI Provider/BLS certification within seven days of hire or transfer, or before independent patient care, whichever comes first.
Knowledge of occupational therapy materials, equipment, procedures, and treatment techniques.
Strong organizational skills and attention to detail.
Ability to prioritize, think critically, and work independently.
Excellent verbal, written, and interpersonal communication skills.
Ability to handle sensitive and confidential situations with professionalism and compassion.
Working knowledge of Microsoft Office Suite and ability to learn additional software as needed.
Valid driver's license, reliable transportation, and ability to travel to patient residences throughout Flathead County and Lake County.
