Position Summary Responsible for assessment, counseling and therapeutic interventions to individuals, families, and groups. The counselor collaborates with primary care providers, psychiatric staff and other multidisciplinary team members to support whole-person care and improve patient outcomes. Principal Accountabilities Standards of Performance: Respect, Integrity, Compassion, Collaboration, Stewardship, Accountability, Quality Education Master's Degree in Social Work or Counseling Licenses and Certification LCSW or LCPC in Illinois Experience and Skills Technical Experience: 2 years of experience in providing individual, family and/or group therapy required. Strong interpersonal and communication skills In-depth knowledge of community resources Skill in diagnostic formulation (DSM-5) Proficiency with EHR systems and documentation standards Knowledge of trauma-informed and recovery-oriented care. Physical Activities Intermittent hand manipulation required Intermittent lifting and carrying of 20 pounds Role Specific Responsibilities Conducts comprehensive biopsychosocial assessments, diagnostic evaluations, and treatment planning in accordance with clinical standards Provides individual, family, and/or group therapy for patients experiencing behavioral, emotional, or mental health challenges Collaborates with medical providers, psychiatrists, and advanced practice providers to coordinate care and ensure continuity of treatment. Provide crisis intervention, risk assessments, and safety planning when indicated. Support patients with referrals to a higher level of care or community resources when appropriate. Maintain compliance with HIPPA, ethical guidelines, and all state/federal laws governing clinical practice. Utilizes evidence-based practice standards to guide the evaluation of care, course of treatment, and appropriate use of organizational and patient-centered resources. Develops and maintains collaborative relationships with organizations in the community that facilitate efficient and effective planning for continued care for the patient. Collaborate with the patient, necessary staff, and post-discharge care providers to assure a safe and effective discharge plan.
