This responsibility will also focus on those patients who are uninsured or underinsured. This work entails individual patient care coordination as appropriate and collaboration with other health care professionals and pharmacy team members. This coordinator will ensure patients have access to resources, services, affordable medication and equipment to increase their health, independence and reduce the need for inpatient hospitalization and Emergency Department use. Serves as an important link between patient, healthcare teams, payers and the community. EDUCATION: High School/GED plus one year of post high school education preferred or related work experience considered PTCB/NHA Certification encouraged/preferred Licenses: Vermont Board of Pharmacy Technician in good standing required
