Provide brief, targeted behavioral health interventions to help patients make meaningful changes
Offer behavioral support strategies that empower patients to manage chronic illnesses and improve overall health
Collaborate with primary care teams to address behavioral health needs in real-time
Share knowledge and educate patients using tailored, easy-to-understand resources
Be a vital part of the clinic's mission by supporting a team-based care approach
Deliver direct care and consultation that meets all professional and program standards
Prioritize safety, quality, and inclusivity in every interaction
Ensure care is always age-appropriate and patient-centered
Bring a friendly, approachable attitude to every patient, visitor, and colleague
Stay sharp and informed-keeping up with all compliance, licensing, and ethical standards
Requirements
A Master's Degree in Social Work or equivalent education/experience combo
Current licensure as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
At least 3 years of experience in a healthcare setting
Strong communication skills (both written and verbal)
Ability to read and speak English fluently
